Rehabilitation Clinic Referral Guide and Form

Health Professionals may refer clients to the Campus Rehab Clinic by several methods.

Once your referral is received, the Rehab Clinic will contact the client to schedule an initial evaluation session. The referring health professional may also be contacted for further consultation. The Rehab Clinic faculty have the right to refuse a patient who, in their opinion, would not serve the educational purposes of the College.

Priority will be given to those patients (clients) who best meet the educational mission of the rehabilitation clinic as determined by the service provider.

The Rehab Clinic is ideal for patients who have run out of insurance benefits or who have never had insurance available to them. While the clinic does not charge fees, donations are always welcome to help offset the cost of supplies.
